WHEREAS, the City Plenning Commission did hoid a public hearing at the City Hall in the Gity of Aneheim on
April 13~ . ~964o et 2:00 o'dock P.M., notice of said public hearing having been duly given as requiced by
law end in accordence with the proviaions oithe Aneheim Municipal code, Chepter 18.64, to hear end conaidet evidence
for end egainst said pcoposed conditioael use end to investiget~ aad make fiadings and recommendetioas in connection
therewith; and •

2. That the proposed nae is pinperly one for which a^„onditionat .Use Permit is authorized by this Code, :o

of the Citizens of the City of Meheim. .
4. That the granting of the Conditionel Use Pemtit under the conditions imposed, if any, will not he detrimental
to the peace, health, safety, and general welfare of the Citizens of the City of Anaheim.
5. That no one appeared in opposition to subject petition.

NOW, TFYEREFORE, BE IT RESOLVED that the Anaheim City Planning Commission iloes hereby grant subject
Petition for Conditional Use Permit, upon the following conditions which are heteby found to be a necessary pcerequisite
to the proposed use of the subject property in order to pceserve the safety and gene:al welfare ofthe Citizens of the '
City of Artaheim:
1. The the owners of subject property shall pay to the City of Maheim the sum of $2.00'
per front foot along ~tate.College Boulevard.
2. That street improvement plans shall bc prepared and a1l.engineering requiremenis of
the City of- Anaheim along State College Boulevard, such as curbs and gutters, sidewalks, street
grading and ~aving, drainage facilities, or other appurtenant work shall ba completed as requiied
by the Citv ngineer and~irr accordance with standard~plans and specifications on file in the
office• of •the City Engineero
3. That the owners of subject property sha11 pay to the City of Anaheim the sum of 15~
pex front foot along..State College Boulevazd, for tree planting purposeso
4. 'Fhat Condition Nos. 1, 2, and 3y above mentioned, shall be complied with prior to the
time that the Building Permit is issued or within a period of 180 days from date hexeof, which-
ever occurs first, or such further time as the Commission may grant.
5o That trash storag~ areas shall be provided in accordance with approved plans on file
in the office of the Director of Public Workso
6~ That subject property shall be developed substantially in accordance with plans and
specifications on file vvith the City of Anaheim, marked Exhibit Nc 1, provided that the clinic
shall be setback a distance equal to the existing building setback on the abuttinq properties
to the south, ana furiner provided that the wails and ceilings shall reflect the average
decibils of sound as indicated on the plan, or shall be determined to contain adequate sound-

7.That a b-foot masonry wall shall be constructed along the south and~east propertY lineso
So That Condition Nos~ 5 and 7, above mentioned, shall be complied with prior to final
building inspection.
9o That a bond in an amount and form satisfactory to the City of Anaheim shall be posted
with the City to guarantee the installation of a 6-foot masonry wall along the north property
line, said bond to be posted rior to the time that the Buildinq Permit is issued or within a
period of 1Fi0 uays from date ~ereofy whichever occurs firsty or such further tiR~e as the
Commission may grant.
