A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM AMENDING THE RATES, RULES AND
REGULATIONS FOR THE SALE AND DISTRIBUTION OF
ELECTRIC ENERGY AS ADOPTED BY CITY COUNCIL
RESOLUTION NO. 71R -478 AND AMENDED BY CITY
COUNCIL RESOLUTION NOS. 73R -25, 73R -373,
73R -531, 74R -75, 74R -630, 75R -243, 75R -344,
75R -345, 76R -41, 76R -377, 76R -528, 76R -817
77R -464 AND 77R -628.
WHEREAS, the City of Anaheim maintains an electric
distribution system for the furnishing of electricity to
residents and inhabitants of the City of Anaheim; and
WHEREAS, it is the policy of the City Council that
Anaheim's Electric Utility shall be self - supporting from
charges made by the City for electric energy furnished to
consumers and for services rendered in connection therewith;
and
WHEREAS, the Public Utilities Board has recommended
establishment of a Service Establishment Charge of $5.00,
increasing the Reconnection Charges from $2.00 to $12.00 and
establishment of an additional charge of $5.00 for same day
or "after hours" service; and
WHEREAS, such charges are designed to offset the
costs of establishing and reconnecting electric service.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the Electric Rates, Rules and
Regulations adopted by City Council Resolution No. 71R -478 and
amended by City Council Resolution Nos. 73R -25, 73R -3732
73R -5312 74R -75 74R -630, 75R -243, 75R -344, 75R -345, 76R -41,
76R -377, 76R -526, 76R -817, 77R -464 and 77R -628 be amended as
follows, to wit:

RULE NO. 3
APPLICATION FOR SERVICE
A. APPLICATION FOR SERVICE
Each applicant for utility service may be required to sign an
application on a form provided by the Utility and, upon request,
will be required to furnish the following information:
1. Name of applicant.
2. Date and place of application.
3. Location of premises to be. served.
4. Date applicant will be ready for service.
5. Whether the premises have been heretofore supplied.
6. Purpose- for which service is to be used.
7. Customer's mailing address.
8. Whether applicant is owner or tenant of, or agent for,
the premises.
9. Rate schedule desired if optional rate is available.
10. Information to establish credit of applicant.
11. Such other information as the Utility may reasonably
require.
The above information may be supplied by the applicant either in
writing or by telephone if the applicant's signature i*s not
required.
The application is a request for service and does not in itself
bind the Utility to serve except under its filed tariff
schedules, nor does it bind the customer to take service for a
longer period than the minimum requirements of the rate schedule.
B. INDIVIDUAL LIABILITY FOR JOINT SERVICE
Two or more persons who join in one application or contract for
service shall be jointly and severally liable thereunder and
shall be billed by means of a single periodic bill.mailed to
the person designated on the application to receive the bill.
C. CHANGE IN CUSTOMER'S EQUIPMENT OR OPERATIONS
Customers shall give the Utility written notice of the extent
and nature of any material change in the size, character, or
extent of the utilizing equipment or operations for which the
Utility is supplying service before making any such change.

D. SERVICE ESTABLISHMENT CHARGE
1. For each establishment of utility service, the Utility
will charge $5.00.
2. The service establishment charge provided for herein is
in addition to the charges in accordance with the
applicable schedule and may be made each time an account
is established. As used herein.. establishment means
each time an account is opened, including a turn on of
utility service or a change of name which requires. a
meter reading.
3. In case the customer requests that utility service be
established on the day of his request or outside of
regular business hours, an additional charge of $5.0
will be made.
4. The service establishment charge is not applicable by
customers of the Utility to service rendered through
submeters to tennants..

at least 5 days, he has not complied with the notice. The
Utility may dispense with the giving of such notice in the
event there exists in the Utility's opinion a dangerous
condition, thus rendering the immediate discontinuance of
service to the premises imperative.
H. CUSTOMER'S REQUEST FOR SERVICE DISCONTINUANCE
When a customer desires to terminate his responsibility for
service, he shall give the Utility not less than two days..'
notice of his intention and state the date on which he wishes
the termination to become effective. 'A customer may be held
responsible for all service furnished at the premises until
two days after receipt of such notice by the Utility or until
the date of termination specified in the notice, whichever
date is later.
I. RESTORATION -- RECONNECTION CHARGE
The Utility will require payment of a reconnection charge of
$12.00 for each service disconnected before restoring service
that has been discontinued for nonpayment of bills or for
failure otherwise to comply with tariff schedules.
In case the customer requests that such service be reconnected.
on the day of his request or outside of regular business
hours, an additional charge of $5.00 will be made.

BE IT FURTHER RESOLVED by the City Council of
the City of Anaheim that the amended Electric Rates, Rules
and Regulations shall go into effect July 19, 1978.
THE FOREGOING RESOLUTION is approved and adopted
by the City Council of the City of Anaheim this 18th day
of July, 1978.
