A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM, CALIFORNIA, APPROVING A
MEMORANDUM OF UNDERSTANDING WITH ANAHEIM CITY
SCHOOL DISTRICT TO ESTABLISH THE HEALTHY
START SERVICE CENTER AT PALM LANE SCHOOL.
WHEREAS, the Anaheim City School District has applied
to the California Department of Education for a grant in the
amount of $50,000 for the purpose of establishing at Palm Lane
School an Integrated Service System to be called the Healthy
Start Service Center; and
WHEREAS, the Integrated Service System would serve
Thomas Jefferson, Francis Scott Key, Palm Lane and Paul Revere
Schools, grades K-6; and
WHEREAS, representatives of the Anaheim City School
District and of the Library Department of the City of Anaheim
have prepared a Memorandum of Understanding setting forth certain
services to be provided by each entity in connection with the
grant program, and a copy of that. Memorandum of Understanding is
attached hereto and incorporated by this reference herein; and
WHEREAS, the public intere~',t would be served by
providing the services described in the Memorandum of
Understanding to the four schools designated above;
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim that the attached Memorandum of Understanding
between the Anaheim City School District and the Anaheim Public
Library be, and the same is hereby, approved.
BE IT FURTHER RESOLVED that the Library Director be,
and he is hereby, authorized to execute said Memorandum of
Understanding on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the

MEMORANDUM OF UNDERSTANDING
INTEGRATED SERVICES
This memorandum of understanding is made and entered into by and between the
ANAHEIM CYFY SCHOOL DISTRIC'F and A,aheim Public Libratad for the purpose of
establishing an h~tegrated Service System to be called the Healthy Start Service Center at
Palm Lane School.
The primary goal of the Healthy Start Service Deliw;ry System is to produce measur-
able improvement in outcomes for stmlet~ts and their families in the areas of school
attendance and performance, physical., social and mental health and family functioning.
This proposal establishes a school~based service team with full participation from the
public and private sectors. The objectives of the collaboration are to:
1. Develop a shared case manag~ment system that meets a variety of needed and
identified services;
2. Improve communication, platoring, coordination, collaboration and cooperation
in the delivery of services;
3. Develop innovative service delivery concepts addressing the specific and mul-
tLple needs of families;
4. Provide prevention and early intervention in drug alcohol abuse, child abuse
prevention, and children's health services;
5. Coordinate the assistance to IEose families receiving multiple services
6. Identify gaps and duplication of services;
7. Identify additional resources available to children and their f,uniEes and ir'~clud,,:.

If dxe grant is awarded to the Anaheim City School District, the Anaheim Public
brary agrees to work in collaboratio~ with the four targeted schools, the Sc~ool Die. der

The Anaheim City School District agrees to:
Provide training for school slaff and collaborators in the project activities,
2. Assist in developing the referral process and provide training to school staff in
its use,
3. Provide information to the community on the Healthy Start servicers,
4. Maintain and safeguard all confidential informa lion obtained as part of this
agreement and,
5. Assist in the translation of materials for parents who are limiteel in English
language proficiency.
This agreement can be voided by either party upon 30 days written notice.
