GENERAL EMPLOYEES
and the "
CITY OF ANAHEIM
The Anaheim Municipal Employees Association (AMEA), General Employees, and
the City of Anaheim (ANAHEIM) have met and conferred and have reached an
agreement on the implementation of the recommendations made by the 3oint
Committee on Medical Programs to the Human Resources Director. The PARTIES have
reached an understanding to amend certain sections of Article 50 -- Insurance, to be
effective .lanuary 1, 2001, as follows:
ARTICLE 50
INSURANCE
50.1 Health Insurance
50.11 ANAHEIM agrees to offer the Kaiser FoundaUon Health Plan and other prepaid
and/or insured health plans recommended by the .ioint Committee on Medical
P.rograms and approved by the City Coundl. Health Maintenance Organization
(HMO) benefit schedules shall be standardized to the extent possible, and shall
require a {,~ $10 co-payment for outpatient and physician services, .'.rd ~r

50.1211 100% of the monthly rate of all approved HMO plans
