A R~SOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM AUTHORIZING PROPOSED SUPPLEMENT
NO. i MODIFYING CONTRACT NO. PH 86-64-48,
RELATING TO A I~DICAL SELF-HELP PROGRAM,
BETWEEN THE CITY OF ANAHEIM AND THE UNITED
STATES DEPARTMENT OF HEALTH, EDUCATION, AND
WELFARE.
WHEREAS, on the lSth day of November, 1963, the City
of Anaheim and the United States Department of Health, Educa-
tion, and ,~elfare, entered into Contract No. PH 86-64-48, for
preparatio~ cf television tapes to be used to promote public
awareness of the medical self help program; and
· .,%I~R~,~'' .... it i~as 'been found advantageous and in the
~est interests of n'he Governmenn to modify said contract to ex-
tend tme period of service at no additional cost.
NOW, THEREFORE., BE IT RESOLVED by the City Council
of Che City of Anaheim that said contract be modified in the
following particulars, 'but in no others:

reading ~June 15, 1964?' is hereby changed
to read "December 15, 1964.~
BE IT FURTHER RESOLVED that ti~e Mayor and the City
Clerk be, and they are hereby authorized to sign said Supplemen-
tal Agreement modifying Contract No. PH 86-64-48 for on behalf
of the City of Anaheim.
