A RES<PLUTION~ THE CITY COUNCIL OF THE CITY
OF AN.HEIM APP.OVING THE TERMS AND CONDITIONS
OF AN' AGREEME WITH SUPPLEMENTAL DENTAL PLANS,
INC. .urn AUTHO$IZING AND DIRECTING THE MAYOR
AND C!TY CLERK' TO EXECUTE SAID AGREEMENT.
WHERE"'S, the City of Anaheim 1s in need of dental
services for t~ .eniploye,'s; and
WHERE.~S, an agreement ,entitled "Agreement For
Dental Health ptogram" (~ereinafter "Agreement") has been
prepared whereby' Supplem,ntal Dental Plans, Inc. (formerly
"James H. Kaufman, D.D.S., Inc.") will provide such dental
services; and
WHERE.B, thet,rmsand conditions of Agreement
are acceptable~o the City Council of the City: of Anaheim.
NOW, THEREFORE. BE IT RESOLVED that .theterms and
conditions of said Agree~ent are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk be, and tpeyare h~reby, authorized and directed to
execute said Agreement o~ behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted
by the City Council of the City of Anaheim this 29tb day
