A RESOLutION OF THE CITY COUNCIL OF THE CITY
OF ANAHEtM APPROVING AN AMENDMENT TO THE
AGREEMENt OF APRIL 19, 1977, FOR EMPLOYEE
DENTAL c.tm BY AND BETWEEN THE CITY OF
ANAHEIM AnD CALIF0RNIA MEDICAL GROUP HEALTH
PLAN, IN~. AND AUTHORIZING THE PERSONNEL
DIRECTORfO EXECUTE SAID AMENDMENT TO AGREE-
MENT ON BEHALF OF THE CITY OF ANAHEIM.
WHEREAS, i,-n agreement for medical care services was
entered into byan<J.:between the City of Anaheim and California
Medical Group Heal~h Plan, Inc. on April 19, 1977; and
WHEREAS, the parties to said agreement desire to amend
said agreement; and
WHEREAS, an amendment to said agreement has been pre-
pared, the terms and conditions of which are satisfactory to the
City of Anaheim.
NOW, THE~$FORE, BE IT RESOLVED by the City Council of
the City of Anahei}$ that the terms and conditions of an amendment
to the agreement ot April 19, 1977, by and between the City of
Anaheim and Califo~rtia Medical Group Health Plan, Inc. be, and
the same are hereby, approved.
BE IT FURTHER RESOLVED that the Personnel Director be
authorized and dir~4ted to ~xecute said amendment to agreement
for and on behalf dt the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the
City Council of the! City of Anaheim this 19th day of SePtember ,
1978.
