RES()LUTION NO. 78R- 826
A RESOLUT ON OF THE CITY COUNCIL OF THE
CITY OF ElM APPROVING THE TERMS &
CONDITION OF ADDENDUM NO. 1 TO THAT
CERTAIN A REEMENT !:NTERED INTO MAY 2,
1978, WI NORTHWESTERN NATIONAL LIFE
INSURANCE! COMPANY AND AUTHORIZING THE
MAYOR AND! CITY CLERK TO EXECUTE SAID
ADDENDUM.
WHEREAS, fy Resolution No. 78R-265 dated May 2,
1978, .the City cou.nil ap. pro". ed the terms of an Agreem.ent
with Northwestern N tionalL1feInsurance Company (NWNL),
wherein NWNL agreed, to administer a Total and Permanent
Disability Benefit (TPD) on a cost p1usbasisj and
WHEREAS, ~he terms of said Agreement, including
a Policy Administration Charge, were submitted to the
City in a letter da~ed January 17, 1978; and
WHEREAS, the Agreement document submitted by
NWNL and approved bY the City Council inadvertently
omitted the monthly'Policy Administration Charge of
$175; and
WHEREAS, 1\.ddendum No. 1 has been prepared to
cover said Policy AJ:1ministration Charge, the terms of
which are acceptab1fa to the City Council of the City
of Anaheim.

. NOW, THE.ORE, BE. IT RESOLVED by the Citr
Counc~l of the Cityrof Anahe~m that the terms of sa~d
Addendum No. 1 are hereby approved and accepted.
BE IT F~. ER RESOLVED that the Mayor and
City Clerk be, and hey are hereby, authorized to
execute said Adden on beb;a1f of the City of Anaheim.
