OF ANAHEIM PPROVING AN AGREEMENT WITH HMO
CONCEPTS, I,C. PERTAt,NING TO DENTAL SERVICE
FOR EMPLOYE S, AND AUTHORIZ ING THE PERSONNEL
DIRECTOR TO,tXECUTE SAID AGREEMENT.
WHEREAS, th, City Council of the City of Anaheim is
desirous of providingLa.dditional medical-and dental benefits for
the employees of the ~ity; and
WHEREAS, HMP" Concepts, Inc., a federally qualified
health maintenance or~anizatioh, has submitted a Group Dental Service
Agreement; and
WHEREAS, th~ terms and conditions of said Agreement are
satisfactory to the C~t:y of Anaheim; and
NOW, THERE~' , BE IT RESOLVED by the City Council of
the City of Anaheim t " t the terms and conditions of the Group
Dental Service Agree t betwe,en the HMO Concepts, Inc. and the
City of Anaheim be, a d the same are hereby, approved.
BE IT FURT~R RESOLVED that the Personnel Director is
authorized to execut~ said Agreement for and on behalf of the
City of Anaheim. J
