A RESOLUTIO OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM PPROVING A RENEWAL AGREEMENT WITH
F AMIL Y HEAL PROGRAM FOR EMPLOYEE HEALTH
SERVICES' AUTHORIZING THE PERSONNEL DIRECTOR
TO EXECUTE AID RENEWAL AGREEMENT ON BEHALF OF
THE CITY OF ANAHEIM TO BE EFFECTIVE JANUARY 1,
1979.
WHEREAS, th~City of Anaheim and Family Health
Program entered into . agreement dated,Apri1 12, 1977,
whereby Family Health Program provided health services
for employees of theity; and

WHEREAS, Fa.~' .ly Health Program has submitted a
Renewal Agreement to he City to provide for such health
services effective Ja uary 1, 1979; and
WHEREAS, th~ terms and conditions of the Renewal
Agreement are acceptaple to the City Council of the City of
Anaheim. '
NOW, THEREFj)RE, BE IT RESOLVED that the terms and
conditions of said agreement are hereby approved and accepted.
BE IT FURT~ RESOLVED that the Personnel Director
be, and he is hereby,! authorized to execute said Agreement
on behalf of the Cityl of Anaheim.
THE FOREGOIr RESOLUTION is approved and accepted
