A RESOLU ION OF THE CITY COUNCIL OF THE CITY
OF ANAHE M APPROVING AN ApffiNDMENT TO THE
AGREEMEN OF MAY 10, 1977, FOR EMPLOYEE MEDICAL
AND HOSP TAL CARE BY AND BETWEEN THE CITY OF
ANAHEIM ND KAISER FOUNDATION HEALTH PLAN, INC.,
AND AUTH RIZING THE PERSONNEL DIRECTOR TO
EXECUTE AID AMENDMENT TO AGREEMENT ON BEHALF
OF THE C TY OF ANAHEIM.

WHEREAs,;an agreement for group medical and
hospital services as entered into by and between the City
of Anaheim and Kai r Foundation Health Pla.n, Inc. on May 10,
1977; and :
WHEREAs,jthe parties to said agreement desire to
amend certain prov~sions thereof; and
WHEREAS ,~jan amendment to said agreement has been
prepared, the term and conditions of which are satisfactory
to the City of Ana im.
NOW, THE:JFORE, BE IT RESOLVED by the pity Council
of the City of An im that the terms and conditions of a
1979 amendment to e agreement of May 10, 1977, by and
between the City 0 ,Anaheim and Kaiser Foundation. Health Plan,
Inc. be, and the s+e are hereby, approved.
BE IT FU HER RESOLVED that the Personnel Director
be, and they are heeby, authorized to execute said amendment
to agreement for on behalf of the City of Anaheim.
THE FOREG ING RESOLUTION is approved and adopted by
the City Councilor the City of Anaheim this 12th day of
December , 1& 7 8 .
