A RESOLUTIO~'. IOF THE C.ITY COUNCIL OF THE CITY.
OF ANAHEIM PROVING AN AMENDMENT TO AGREEMENT
BETWEEN THE. ITY OF ~AHEIM AND THE CALIFORNIA
DENTAL SERVJCE A~D AUTHORIZING THE PERSONNEL
DIRECTOR TO! ~XECUTE SAID AMENDMENT.
WHEREAS, put~uant to Resolution No. 73R-578, the
C.ity of Anaheim enter~.~. into an. agreement with the California
Dental Service to emp~py a dental health care plan for certain
City employees; and f' .
WHEREAS, thklParties to said Agreement desire to amend
said Agreement in ord. r. to pen. it City auditors to audit the
records of the Ca1ifo . ia Dental Service; and
WHEREAS, an~endment to said Agreement has been pre-
pared, the terms and ~bnditions of which are satisfactory to
the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim t~~t the Amendment to that certain Agreement
between the City and California Dental Services this day submitted
be, and the same is h~reby, approved.
BE IT FURTH$R RESOLVED that the Personnel Director is
hereby authorized to ~~ecutethe said Amendments to Agreement
for and on behalf of the City of Anaheim.
THE FOREGOI~~ RESOLUTION is approved and adopted by
the City Council of ttt~ City of Anaheim this 12th day of
December , 197L.
