(hereinafter "Northwe .ern") has issued Group Life Insurance
Policy GL-11742_l to e City of Anaheim, and
WHEREAs, the fOlicy anniversary date is January 8,

WHEREAS. the ria.. rUes desire to amend said anniversary
date to January 1 for ejonvenience and easier administration,
and
WHEREAs, an -ndment has been prepared, the terms and
conditions of which areflicceptable to the City Council of the
City of Anaheim. !
NOW, THEREFORE,! BE IT RESOLVED that the terms and
conditions of said amen"""'nt are hereby approved and aCcepted.
