WHEREAS, Nor11Iwestern National Life Insurance Company
(hereinafter "Northwe tern") has issued Group Life Insurance
Policy GL-11742~1; an
WHEREAS, it is desirable to renew said policy for
1979 with no change in premium rates; and
WHEREAS, the hnedical ~lan Aggregate Excess Risk
Insurance protectsth~ City against inordinately excessive
experience for a year~ and
WHEREAS, ~t \$ desirable to renew the medical plan
Aggregate Excess R~skltnsurance for 1979 at the threshold of
l25~ of $110.89 per e~loyee per month for an annual premium
of $3,000.
NOW, THEREFOtE, BE IT RESOLVED that Group Life
Insurance Policy GL-lt742;"1 be, and it is hereby, renewed
for the 1979 calendar/year with no changes in premium rates.
BElT FURTHE~ RESOLVED that the medical plan
Aggregate Excess RisklInsurance for 1979 be, and it is
hereby renewed, and tije payment of an annual premium of
$3,000 therefor is heJteby authorized.
THE FOREGOINi RESOLUTION is approved and adopted
