Population to be Served: CONTRACTOR shall provide the services to promote safe and stable families as specified below to at risk families with children ages 0 - 18, who reside in Anaheim, California. 2. Scope of Work: CONTRACTOR shall provide services, as described in Paragraphs 3 through 9 below, to address one or more of the following FaCT goals: 2.1 To promote safe and stable homes for efforts aimed at child abuse, domestic violence. abuse, hereinafter referred to as Goal #1. children through prevention gang activity, and substance 2.2 Increase accessibility to health and mental health care including preventive health care and substance abuse treatment, hereinafter referred to as Goal #2. 2.3 Increase availability and accessibility of quality child care and youth programs, hereinafter referred to as Goal #3. 2.4 Assist families in obtaining access to resources so that they may achieve economic self-sufficiency, hereinafter referred to as Goal #4. 2.5 Strengthen parenting skills among biological, kinship, foster and adoptive parents, and blended families, hereinafter referred to as Goal #5. 2.6 Assist students in achieving success at school, hereinafter referred to as Goal #6. 1 of 19 1 2 3 4 5 6 7 8 9 10 I1 12 13 14 15 16 17 18 19 20 21 22 23 24 25 26 27 28 2.7 Build the capacity of communities to address the needs of children, youth and families, hereinafter referred to as Goal #7, 3. Goal #1 and Related Activities/Services: Goal #1: To promote safe and stable homes for children through prevention efforts aimed at child abuse, domestic violence, gang activity, and substance abuse. 3.1 CONTRACTOR SHALL PROVIDE THE FOLLOWING CASE MANAGEMENT SERVICES: 3.1.1 CONTRACTOR shall provide services to children and families residing in the target area, hereinafter referred to as "participants." 3.1.2 CONTRACTOR shall provide Case Management Services including, but not limited to, information and referral, public utilities assistance, food distribution, para-professional counseling, transportation assistance and education assistance services, to five hundred (500) participants by tracking current services for each participant and referring participants to other identified resources within the community. 3.1.3 CONTRACTOR shall provide services daily from Monday through Friday from 8 a.m. to 5 p.m. CONTRACTOR shall provide services to participants who telephone or walk in to either of the FRCs. CONTRACTOR shall provide a phone messaging system to record messages during all other times. 3.1.4 CONTRACTOR shall provide services at Ponderosa Park Family Resource Center (FRC) located at 2100 S. Haster Street, Anaheim, CA 92802 and at Manzanita Park FRC located at 1260 Riviera Street, Anaheim, CA 92801. 3.1.5 CONTRACTOR shall measure progress by ensuring that the number of participants shall be recorded on a tracking log along with the following information: date and time of service rendered; name of staff member or volunteer providing the service: time when a phone message was returned: indication if follow-up is necessary; nature of service provided; how participant heard about the FRC: name, address, and phone number of 2 of 19
