A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A RIDER TO BE ATTACHED
TO AND FORM A PART OF GRO!JP LIFE POLlCY NO.
G-6269 OF THE TRAVELERS I~SURANCE COMPANY,
HARTFORD, CONNECT I CUT , PRPVID ING Fffi INSURANCE
AGAINST LOSS OF LIFE OF DEPENDENTS OF CITY
EMPLOYEES.
WHEREAS, The Travelers Insurance Company of
Hartford, Connecticut has heretofore issued its Group Life
Insurance Policy No. G-6269 insuring employees of the City
of Anaheim as therein provided, which policy is now in full
force and effect; and
WHEREAS, said Travelers Insurance Company has
submitted to the City of Anaheim and the employees of the
City of Anaheim a rider to be attached to and form a part
of said Group Life Policy No. G-6269 to insure said City
employees against loss of life of their dependents; to the
extent and in the manner as provided in said rider; and
WHEREAS, the City Council is informed and advised
that said rider is satisfactory to the employees of the
City of Anaheim, and does further find that the attaching
of said rider to said Policy No. G-6269 insuring against
loss of life of dependents of said employees would be for
the best interest of said employees of the City,
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that said rider to be attached to
and form a part of Group Life Insurance Policy No. G-6269
issued by The Travelers Insurance Company of Hartford, Con-
necticut to the City of Anaheim, insuring employees insured
under said policy against loss of life of their dependents
as provided in said rider, is hereby approved and authorized
to be attached to and become a part of said pblicy, and be
made available to employees of the City eligible for such
insurance and who wish to avail themselves of it.
