A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING THE TERMS AND CONDITIONS
OF AN AGREEMENT WITH CALIFORNIA VISION
SERVICE, DBA VISION SERVICE PLAN, AND AUTHOR-
IZING AND DIRECTING THE MAYOR AND CITY CLERK
TO EXECUTE SAID AGREEMENT. (Vision Care
Benefits)
WHEREAS, the City of Anaheim desires to provide
vision care benefits to eligible employees and their
families; and
WHEREAS, California Vision Service, a California
non - profit corporation doing business as Vision Service
Plan, is qualified to provide services to implement such
benefits by providing a panel of doctors to perform services
to covered persons under certain terms and conditions; and
WHEREAS, an agreement, entitled "Group Vision Care
Agreement" (hereinafter "Agreement ") has been prepared
whereby California Vision Service will provide such services
to the City of Anaheim; and
WHEREAS, the terms and conditions of Agreement are
acceptable to the City Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said Agreement are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk be, and they are hereby, authorized and directed to
execute said Agreement on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted
by the City Council of the City of Anaheim this 11th day of
April , 1978.
