A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING THE TERMS AND CONDITIONS
OF AN AGREEMENT WITH NORTHWESTERN NATIONAL LIFE
INSURANCE COMPANY IN CONNECTION WITH DISABILITY
" INSURANCE AND AUTHORIZING THE MAYOR AND CITY
CLERK TO EXECUTE SAID AGREEMENT.
WHEREAS, Northwestern National Life Insurance Company
(hereinafter "Northwestern ") has issued Group Life Insurance
Policy GL- 11742 -1 to the City of Anaheim which provides benefits
including a total and permanent disability benefit; and
WHEREAS, Northwestern and the City of Anaheim desire to
provide for a retroactive determination of premiums payable
with respect to the total and permanent disability benefit made
available to those individuals identified in the schedule of
benefits of said Policy GL- 11742 -1; and
WHEREAS, an agreement has been prepared, the terms and
conditions of which are acceptable to the City Council of the
City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said Agreement are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Mayor and City Clerk
be, and they are hereby, authorized to execute said Agreement
on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved
