Month 40 Storekeeper, Assistant 266.00 40 Yardman 1.41 Hour 40 BE IT FURTHER RESOLVED that in addition to the compensation above set forth that the employees of said City who are insured under a group life insurance plan, heretofore established by said City, shall receive additional compensation! equal to the premium paid upon their respective lives by said City which additional compensation shall however not be paid directly to the employees but to the coma ;pny carrying such insurance and for such premium; BE IT FURTHER RESOLVED that the City of Anaheim shall deduct the sum of :seventy cents (700) each month from the salary of such employees insured as out- lined in the foregoing paragraph, which deduction shall be such employeets monthly contribution on the premium for such insurance; 1 BE IT FURTHER RESOLVED that in addition to the compensation above set forth that the regular employees of said City are members of the hospital Service 2 of Southern California and California Physicians' Service and shall receive additional compensation equal to the monthly charge for such service for each of 3 such City Employees which additional compensation shall however, not be paid directly to the employee but to the Hospital Service of Southern California and 4 California Physicians' Service; 5 BE IT FURTHER RESOLVED that the City of Anaheim shall make deductions of authorized amounts from employee's salary payable to the Hospital Service of 6 Southern California and California Physicians Service in payment of premium for 7 dependents; BE IT FURTHER RESOLVED that the City of Anaheim shall make deductions of authorized amounts from employees' salary payable to the Aetna Casualty and Surety Compnay in full payment of premium; 8 9 CITY OF ANAHEIM 204 E. C.ALli"ORN }A 1� BE IT FURTHER RESOLVED that all applicants for position upon the Police and Fire Departments of said City, and all other applicants for positions with 1 the City as the City Council shall from time to time designate, shall success- fully undergo a physical examination, the character of which shall be in the form approved by the City Council of July 1, 1941, or such other form as the Council 12 shall from time to time adopt. Such examination shall be upon said form to be furnished to the applicant by the City Clerk. The City shall not pay to exceed 13 ;Seven Dollars, Fifty Cents ($7.50) for each such examination. No applicant shall !receive said form from the City Clerk nor shall the City be liable for the cost 14 of any examinations, unless the City Council by its order, or a Department Head, by written authorization, shall approve the applicant for such examination. The 15 1physician examining such applicant shall be of a school recognized by the Indust-1 'rial Commission of the State of California, and shall be in active practice with 16 an office in the City of Anaheim; provided, however, that no applicant shall be examined by a physician who has examined or professionally attended either the 17 1 applicant or any members of his family within five years immediately prior to 18 such examination. Subject to the foregoing restrictions, the applicant may be examined by any physician he chooses. Immediately upon completeing such examina -i tion, the examining physican shall deliver the report of such examination upon said form, directly to the City Clerk. 20 21 24 25 26 27 28 29 30 31 BE IT FURTHER RESOLVED that employees of said City shall have the folic ing holidays as vacation with pay: 22 New Year's Day: Lincoln's Birthday: Washington Birthday: Memorial Day: Independence Day: Labor Day: Admission Day: Armistice Day: Thanksgiving Day: 25 Christmas. In the event holidays above mentioned shall fall on Saturday or Sunday the following work day will be taken in lieu of the holiday. BE IT FURTHER RESOLVED that employees receiving compensation on an hourly basis who shall be required to work for said City on the above mentioned holidays, shall receive pay for hours they work on such holidays; and the em- ployees shall receive no pay for any holidays wherein they do not work, other than the holidays above mentioned. AND BE IT FURTHER RESOLVED that all employees, who shall have one year's continuous service immediately proceeding shall be given a vacation of two calendar weeks with pay each year in addition to the holidays above set forth. w■■ AID BE IT FURTHER RESOLVED that vacations must be taken within the calendar year of each year.
