WHEREAS, Hospital Service of Southern California California Physiciansz
Service, a non-profit Corporation, operating under the laws of the State of
California, has offered their plan of prepaid surgical and medical care to
officials and employees of the City of Anaheim; and
WHEREAS, more than 50 per cent of said officials and employees of said
City has signified by the filing of individual application forms their intention
for membership in said Hospital Service of Southern California California
Physicians' Service; and
WHEREAS the City of Anaheim will make required Payroll deductions for
members of the plan of insurance charges under the plan on a group basis.
NOW, THEREFORE, BE IT RESOLVED that the City of Anaheim does hereby
approve the plan and hereby agrees to accept monthly billing on its employees
for insurance charges on a group basis of said insurance charges from the Hospital
Service of Southern California California Physicians' Service.
The foregoing Resolution was approved and signed by me this 25th day
of November, 1947.

I, CHARLES E. GRIFFITH, City Clerk of the City of Anaheim, do hereby
certify that the foregoing resolution was passed and adopted at a regular meeting
of the City Council of the City of Anaheim, held on the 25th day of November, 19474
by the following vote of the members thereof:
AYES: COUNCILMEN: Pearson, Heying, Boney and Van Wagoner
NOES: COUNCILMEN: None.
ABSENT: COUNCILMEN: Pace.
