A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT FOR A FULL
COVERAGE, PRE-PAID DENTAL CARE PLAN BETWEEN
THE CITY OF ANAHEIM AND JAMES H. KAUFMAN,
D.D.S., INCORPORATED.
WHEREAS, pursuant to the Memorandums of Understanding
executed by the City of Anaheim and the Anaheim Municipal
Employees Association, Local 47, the International Brotherhood
of Electrical Workers, the Anaheim Fire Association, and the
City unrepresented employees, the City agreed to sponsor and
contribute up to Ten Dollars ($10.00) per month per family for
dental insurance; and
liHEREAS, pursuant to said agreement, requests for such
plans were made to various dental organizations; and
WHEREAS, James H. Kaufman, D.D.S., Incorporated, has
submitted a dental health care service plan which the City Coun-
cil deems to be the best plan submitted for the City employees;
and
WHEREAS, an agreement has been submitted which the City
Council considers to be in the best interests of the City to
enter into for said "full coverage, pre-paid c"ental care by
James H. Kaufman, D.D.S., Incorporated"; and
WHEREAS, the City Council does hereby authorize the
City Manager of the City of Anaheim to execute an agreement
between the City and James H. Kaufman, D.D.S., Incorporated, to
provide a full coverage, pre-paid dental care plan.
NOW, THEREFORE, BE IT RESOLVED that the agreement
between James H. Kaufman, D.D.S., Incorporated, and the City of
Anaheim, this day submitted be, and the same is hereby approved.
BE IT FURTHER RESOLVED that the City Manager is hereby
authorized to execute this agreement for and on behalf of the
City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by me
this 27th day of November, 1973.
