RESOLUTION NO. 171
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AMENDMENT TO AGREEMENT
BETWEEN JAMES H. KAUFMAN, DDS., INC. AND THE
CITY OF ANAHEIM.
WHEREAS, on the 27th day of November, 1973, the
City of Anaheim entered into a certain agreement for dental
services with James H. Kaufman, DDS., Inc., pursuant to
Resolution No. 73R -577; and
WHEREAS, the parties to said agreement desire to
amend said agreement to provide for prepaid "capitation" pay-
ments; and
WHEREAS, an amendment to said agreement iias been
submitted, the terms and conditions of which are satisfactory
to the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the amendment to agreement between
James H. Kaufman, DDS., Inc., this day submitted be, and the
same is hereby approved.
BE IT FURTHER RESOLVED that the City Manager is
hereby authorized to execute the amendment to agreement for
and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by
me this 9th day of Aprii 1974.
