A RESOLUTION OF THE CIT7 COUMCIZ OE TRE CITY
OF ANAHEIM APPROVING AN AMENDMENT TO AGREE-
MENT BFTWEE,`-_-,T H. K�A-UF-T,' 1,1, D.D.S., INC.
AND THE CITY OF ANAHEIM.
WEEREAS, on the 27th day of November, 1973, the City of
Anaheim entered into a certain agreement for dental services with
James H. Kaufman, D.D.S., Inc., pursuant to Resolution No. 73R-577
and as amended by Resolution No. 74R-171 dated March 9, 1974; and
WHEREAS, the said agreement will expire on December 31,
1975; and
WHEREAS, the parties to said agreement desire to amenC''.
said agreement by extending the term thereof until Decenber 31,
1977; and
WHEREAS ,'an amendment to said agreement has been submit-
ted, the terms and conditions of which are satisfactory to the City
of Anaheim.
NaP, THEREFORE, BE 1T RESOLVED by the City Council of the
City of Anaheim that the amendment to agreement between James H.
Kaufman, D.D.S., Inc., this day submitted be, and the same is
hereby, approved.
BE IT FURTHER RESOLVED that the City Manager is hereby
authorized to executetheamendment to agreement for and on behalf
of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by me
