A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT WITH
KAISER FOUNDATION HEALTH PLAN, INC. PERTAINING
TO HEALTH SERVICE FOR EMPLOYEES, AND AUTHORIZ-
ING THE MAYOR AND CITY CLERK TO EXECUTE SAID
AGREEMENT.
WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical benefits for
the employees of the City; and
WHEREAS, Kaiser Foundation Health Plan, Inc. a
corporation, has submitted a Group Medical and Hospital
Service Agreement; and
WHEREAS, said agreement and face sheet dated
April 29, 1977, benefit schedule, and additional provisions
have been reviewed by the staff of the City, the terms and
conditions of said agreement, face sheet, benefit schedule
and additional provisions which are incorporated into the
agreement are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
Group Medical and Hospital Service Agreement between the
Kaiser Foundation Health Plan, Inc. and the City of Anaheim
be, and the same are hereby, approved.
BE IT FURTHER RESOLVED that the face sheet from
the Kaiser Foundation Health Plan, Inc. dated April 29,
Agreement is hereby approved and incorporated into said basic
agreement as though set out therein in full.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said agreement for and on
behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by
me this loth day of May 1977.
