RESOLUTION NO. 77R- 331
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A LETTER AMENDMENT DATED
APRIL 20, 1977 WITH CALIFORNIA MEDICAL GROUP
HEALTH PLAN, INC. PERTAINING TO THE HEALTH
SERVICES TO EMPLOYEES AND AUTHORIZING THE
MAYOR AND CITY CLERK TO EXECUTE SAID LETTER
AMENDMENT.
WHEREAS, the City Council of the City of Anaheim
did, on the 19th day of April, 1977, enter into an agree-
ment with California Medical Group Health Plan, Inc. for
the providing of additional medical and dental benefits
for the employees of the City; and
WHEREAS, California Medical Group Health Plan,
Inc. has submitted to the City of Anaheim a letter amendment
dated April 20, 1977, which amends the Group Prepaid Dental
Care Contract; and
WHEREAS, the terms of said amendment to said
agreement of April 19, 1977, amends subarticle IV.A on
Subscriber eligibility and adds subarticle VI.C.4 to
clarify that retirees must contribute the full amount of
the Prepayments Fee, with no City contribution on their
behalf; and
WHEREAS, said amendments to said agreement are
satisfactory to the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of
that certain letter amendment to the Group Prepaid Dental
Care Contract dated April 20,, 1977 between the City of
Anaheim and California Medical Group Health Plan, Inc. be,
and the same are hereby, approved.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said letter amendment for
and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed
by me this 10th day of May 1977.
