1977-261A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT WITH HEALTH
CARE ASSOCIATES MEDICAL GROUP, INC. PERTAINING
TO DENTAL SERVICE FOR EMPLOYEES, AND AUTHORIZ-
ING THE MAYOR AND CITY CLERK TO EXECUTE SAID
AGREEMENT.
WHEREAS, the City Council of the City of Anaheim is
desirous of providing additional medical and dental benefits for
the employees of the City; and
WHEREAS, Health Care Associates Medical Group, Inc., a
corporation, has submitted a Group Prepaid Dental Care Contract
Agreement; and
WHEREAS, said Agreement has been reviewed by the staff
of the City, the terms and conditions of said Agreement are satis-
factory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City of
Anaheim to enter into said Agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim that the terms and conditions of the Group
Prepaid Dental Care Contract Agreement between the Health Care
Associates Medical Group and the City of Anaheim be, and the same
are hereby, approved.
BE IT FURTHER RESOLVED that the Mayor and City Clerk are
authorized to execute said Agreement for and on behalf of the City
of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by me
this 19th day of April 1977.
