RESOLUTION NO. 77R- 260
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT, ADDENDUM
AND EXHIBITS A, B, C D AND E WITH DENTAL
HEALTH SERVICES, INC. PERTAINING TO DENTAL
SERVICE FOR EMPLOYEES, AND AUTHORIZING THE
MAYOR AND CITY CLERK TO EXECUTE SAID AGREE-
MENT.
WHEREAS, the City Council of the City of Anaheim is
desirous of providing additional medical and dental benefits for
the employees of the City; and
WHEREAS, Dental Health Services, Inc., a corporation,
has submitted a Group Prepaid Dental Care Contract Agreement; and
WHEREAS, said Agreement, Addendum and Exhibits A, B,
C, D and E have been reviewed by the staff of the City, the terms
and conditions of said Agreement, Addendum and Exhibits, which are
incorporated therein, are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City of
Anaheim to enter into said Agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim that the terms and conditions of the Group
Prepaid Dental Care Contract Agreement, Addendum and Exhibits A,
B, C, D and E between the Dental Health Services, Inca and the
City of Anaheim be, and the same are hereby, approved.
BE IT FURTHER RESOLVED that the Mayor and City Clerk
are authorized to execute said Agreement, Addendum and Exhibits
A, B, C, D and E thereto for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by me
this 19th day of April 1977.
