1977-259A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT WITH
CALIFORNIA MEDICAL GROUP HEALTH PLAN, INC.
PERTAINING TO DENTAL SERVICE FOR EMPLOYEES,
AND AUTHORIZING THE MAYOR AND CITY CLERK TO
EXECUTE SAID AGREEMENT.
WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical and dental
benefits for the employees of the City; and
WHEREAS, California Medical Group Health Plan,
Inc. a corporation, has submitted a Group Prepaid Dental Care
Contract Agreement; and
WHEREAS, said Agreement and Exhibits A and B have
been reviewed by the staff of the City, the terms and conditions
of said agreement and exhibits which are incorporated therein
are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
Group Prepaid Dental Care Contract Agreement between the
California Medical Group Health Plan and the City of Anaheim
be, and the same are hereby approved.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said Agreement and Exhibits
A and B thereto for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by
me this 19th day of April 1977
