A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEIIEdT WITH
CALIFORNIA MEDICAL GROUP HEALTH PLAN, IidC.
PERTAINIPNG TO HEALTH SERVICE FOR EMPLOYEES,
AND AUTHORI,,ING THE MAYOR AND CITY CLERK TO
EXECUTE SAID AGREEI!ENT.
WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical and dental
benefits for the employees of the City; and
WHEREAS, California Medical Group Health Plan,
Inc., a corporation, has submitted a Group Prepaid Medical
Care Contract Agreement to become effective July 1, 1977;
and
WHEREAS, said agreement and exhibits have been
reviewed by the staff of the City, the terms and conditions
of said agreement and exhibits which are incorporated into
the agreement are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
Group Prepaid Medical Care Contract Agreement, and Exhibits
A and B between the California Medical Group Health Plan,
Inc. and the City of Anaheim be, and the same are hereby,
approved.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said agreement and exhibits
for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by
me this 19th day of April 1977.
