A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT WITH SECURITY
HEALTH PLAN PERTAINING TO HEALTH SERVICE
FOR EMPLOYEES, AND AUTHORIZING THE MAYOR AND
CITY CLERK TO EXECUTE SAID AGREEMENT.
WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical and dental
benefits for the employees of the City; and
WHEREAS, Security Health Plan, a non profit
corporation, has submitted a Security Health Plan Master
Group Membership Agreement; and
WHEREAS, said Agreement, Exhibits A, B, C and D,
and Addendum and Clarification thereto, to become effective
July 1, 1977 through December 31, 1978, have been reviewed by
the staff of the City, the terms and conditions of which
are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
Security Health Plan Master Group Membership Agreement between
the Security Health Plan and the City of Anaheim be, and the
same are hereby approved.
BE IT FURTHER RESOLVED that the Exhibits A, 8, C
and D and Addendum and Clarification to the Security Health
Plan Master Group Membership Agreement to become effective
July 1, 1977 through December 31, 1978, are hereby approved
and incorporated into said basic contract as though set out
therein in full.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said Security Health Plan
Master Group Membership Agreement, Exhibits A, B, C and D
and Addendum and Clarification thereto for and on behalf of
the City of Anaheim.
THE FOREGOING RESOLUTION is approved and sip ed by
me this 12th day of April 1977.
