A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AGREEMENT WITH HMO
CONCEPTS, INC. PERTAINING TO HEALTH SERVICE
FOR EMPLOYEES, AND AUTHORIZING THE MAYOR AND
CITY CLERK TO EXECUTE SAID AGREEMENT.
WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical and dental
benefits for the employees of the City; and
WHEREAS, HMO Concepts, Inc., a non profit
corporation (HMOC), has submitted a Group Medical and Hospital
Service Agreement and Amendment No. One thereto; and
WHEREAS, said agreement and amendment to become
effective July 1, 1977, have been reviewed by the staff of
the City, the terms and conditions of said agreement and
amendment thereto which is incorporated into the agreement
are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
HMO Concepts, Inc.'s Group Medical and Hospital Service Agree-
ment and Amendment No. One between HMO Concepts, Inc. and the
City of Anaheim be, and the same are hereby approved.
BE IT FURTHER RESOLVED that the Amendment No. One
amending the HMO Concepts, Inc.'s Group Medical and Hospital
Service Agreement is hereby approved and incorporated into
said basic contract as though set out therein in full.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said agreement and amendment
for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and signed by
me this 12th day of April 1977.
