1977-243WHEREAS, the City Council of the City of Anaheim
is desirous of providing additional medical and dental
benefits for the employees of the City; and
WHEREAS, Family Health Program, Inc., a corpora-
tion, has submitted a Family Health Program Group Membership
Agreement; and
WHEREAS, said agreement and cover letter dated
March 28, 1977, from Larry Pett has been reviewed by the
staff of the City, the terms and conditions of said
agreement and cover letter which is incorporated into the
agreement are satisfactory to the City of Anaheim; and
WHEREAS, it is in the best interests of the City
of Anaheim to enter into said agreement.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of the
Family Health Program Group Membership Agreement between the
Family Health Program, Inc. and the City of Anaheim be, and
the same is hereby approved.
BE IT FURTHER RESOLVED that the cover letter from
Family Health Program, Inc. dated March 28, 1977, from Larry
Pett amending the Family Health Program Membership Agreement
is hereby approved and incorporated into said basic contract
as though set out therein in full.
BE IT FURTHER RESOLVED that the Mayor and City
Clerk are authorized to execute said agreement for and on
behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and sig ed by
