A RESOLUTION OF TEL CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AMENDMENT TO THE TAASTFR
GROUP AGREEMENT FOR EMPLOYEE HEALTH CARE BY AND
BETWEEN THE CITY OF ANAHEIM AID GENERAL MEDICAL
CENTERS HEALTH PLAN, AND AUTHORIZING THE MAYOR

WHEREAS, an agreement was entered into by and
between the City of Anaheim and General Medical Centers
Health Plan on April 13, 1977; and
WHEREAS, an amendment to said agreement was
executed by General _Medical Centers Health Plan on April 18,
1977, and executed by the City of Anaheim on May 10, 1977;
and
WHEREAS, said a :reement and amendment thereto
contained a clerical error wherein the Master Group Agree-
ment Grout Number was stated to be "6627" when, in fact,
said nunber should have read "65523;" and
JHEREAS, an aueridment to said agreement has been
prepared to correct said, error, the tends and conditions of
which are satisfactory to the City of Anaher

of the City of Anaheiu that the terms and conditions of an
amendment to the Master Group Agreement, and amendment
thereto, by and between the City of Anaheim and General
Medical Centers Health Plan, be, and the sane is hereby,
approved.
BE IT FURTHER RESOLV that the Mayor and City
Clerk be, and they are hereby, authorized and directed to
execute said agreement for and on behalf of the City of
Anaheim.
