1977-839WHEREAS, an agreement for dental care services was
entered into by and between the City of Anaheim and Health
Care Associates Medical Group, Inc. on April 19, 1977; and
WHEREAS, the parties to said agreement desire to
amend Page 2 thereof pertaining to Definitions; and
WHEREAS, an amendment to said agreement has been
prepared, the terms and conditons of which are satisfactory
to the City of Anaheim.
RESOLUTION NO. 77R- 839
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING AN AMENDMENT TO THE
AGREEMENT OF APRIL 19, 1977, FOR PREPAID DENTAL
CARE BY AND BETWEEN THE CITY OF ANAHEIM AND
HEALTH CARE ASSOCIATES MEDICAL GROUP, INC. AND
AUTHORIZING THE MAYOR AND CITY CLERK TO EXECUTE
SAID AMENDMENT TO AGREEMENT ON BEHALF OF THE
CITY OF ANAHEIM.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of an
amendment to the agreement of April 19, 1977, by and between
the City of Anaheim and Health Care Associates Medical Group,
Inc. be, and the same is hereby, approved.
BE IT FURTHER RESOLVED that the Mayor and City Clerk
be, and they are hereby, authorized to execute said amendment
to agreement for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by
the City Council of the City of Anaheim this 27th day of
December 1977.
