A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A RENEWAL AGREEMENT WITH
FAMILY HEALTH PLAN, INC. FOR EMPLOYEE HEALTH
SERVICES AND AUTHORIZING THE MAYOR AND CITY
CLERK TO EXECUTE SAID RENEWAL AGREEMENT ON
BEHALF OF THE CITY OF ANAHEIM TO BE EFFECTIVE
JANUARY 1, 1978.
WHEREAS, the City of Anaheim and Family Health
Program, Inc. entered into an agreement dated April 12, 1977,
whereby Family Health Program, Inc. provided health services
for employees of the City; and
WHEREAS, Family Health Program has submitted a
Renewal Agreement to the City to provide for such health
services effective January 1, 1978; and
WHEREAS, the terms and conditions of the Renewal
Agreement are acceptable to the City Council of the City of
Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said agreement are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Mayor and City Clerk
be, and they are hereby, authorized to execute said Renewal
Agreement on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and accepted by
the City Council of the City of Anaheim this 27th day of
December 197 7
