1977-835WHEREAS, an agreement for medical care services was en-
tered into by and between the City of Anaheim and General Medical
Centers Health Plan on April 18, 1977; and
WHEREAS, the parties to said agreement desire to amend
said agreement with respect to Prepayment Fees; and
WHEREAS, an amendment to said agreement has been pre-
pared, the terms and conditions of which are satisfactory to the
City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim that the terms and conditions of a second
amendment to the agreement of April 18, 1977, by and between the
City of Anaheim and General iedical Centers Health Plan be, and
the same are hereby, approved.
BE IT FURTHER RESOLVED that the Personnel Director be
authorized and directed to execute said second amendment to agree-
ment for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the
City Council of the City of Anaheim this 27th day of December
1977.
