A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF 'TAI-iEIIi APPROVING AN LMENDIENT TO THE
AGREEMENT OF APRIL 12, 1977, FOR EMPLOYEE
MEDICAL CARE BY AND BETWEEN THE CITY OF
ANAHEIM AND SECURITY HEALTH PLAN, AND AUTH-
ORIZING THE PERSONNEL DIRECTOR TO EXECUTE
SAID AMENDMENT TO AGREEMENT ON BEHALF OF THE
CITY OF ANAHEIM.
WHEREAS, an agreement for medical care services was en-
tered into by and between the City of Anaheim and Security Health
Plan on April 12, 1977; and
WHEREAS, the parties to said agreement desire to amend
Exhibit D thereof pertaining to Membership Dues; and
WHEREAS, an amendment to said agreement has been pre-
pared, the terms and conditions of which are satisfactory to the
City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council of
the City of Anaheim that the terms and conditions of an amendment
to the agreement of April 12, 1977, by and between the City of
Anaheim and Security Health Plan be, and the same are hereby,
approved.
BE IT FURTHER RESOLVED that the Personnel Director be
authorized and directed to execute said amendment to agreement
for and on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is aoproved and adopted by the
City Council of the City of Anaheim this 27th day of December
1977.
