A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING THE TERMS AND CONDITIONS
OF AN AMENDMENT TO AN AGREEMENT WITH KAISER
FOUNDATION HEALTH PLAN, INC., AND AUTHORIZING
AND DIRECTING THE HUMAN RESOURCES DIRECTOR TO
EXECUTE SAID AMENDMENT.
WHEREAS, the City of Anaheim has heretofore entered
into an agreement with Kaiser Foundation Health Plan, Inc.;
and
WHEREAS, the parties desire to amend said agree-
ment with respect to benefits for maternity care effective
April 29, 1979, by revising the premium structure to become
effective May 1, 1979; and
WHEREAS, the terms and conditions of said Amendment
are acceptable to the City Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said Amendment are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Human Resources
Director be, and he is hereby authorized and directed to
execute said Amendment on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted
by the City Council of the City of Anaheim this 24th day of
April 1979.
