A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHELi APPROVING THE TERMS AND CONDITIONS
OF AN ANENDMENT TO TEE CITY'S LONG TERN DISA-
BILITY POLICY CO BY STANDARD INSURANCE
CO dPANY, AND AUTHORZING AND DIRECTING THE
HUMAN RESOURCES DIRECTOR TO EXECUTE SAID
AMENDMENT. (POLICY AO. 334412)
WHEREAS, the City of Anaheim has a Lan y Term Disability
Insurance Policy written by Standard Insurance Company; and
WHEREAS, it is desirable to make certain amendments with
respect to coverage under said policy; and
WHEREAS, there will be no cost for this benefit modifi-
cation for the balance of this Policy year endin December 31,
1979; and
WHEREAS, an amendment to said policy has been prepared,
the terms and conditions of which are acceptable to the City
Council of the City of Anaheim.
THEREFORE, ER IT RESOLVED that the terms and condi-
tions of said Amendment are hereby approved and accepted.
BE IT FURTHE RESOLVED th.at the H=an 1Niources Director
be, and he is hereby, authorized anC direCted to execute said
Amendment on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the
City Council of the City of Anaheim this 7th da August, 1979.
