A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING THE TERMS AND CONDITIONS
OF AN AMENDMENT TO THE CITY'S LONG TERM DISA-
BILITY POLICY COVERED BY STANDARD INSURANCE
COMPANY, AND AUTHORZING AND DIRECTING TI-IE
HUMAN RESOURCES DIRECTOR TO EXECUTE SAID
AMENDMENT. (POLICY NO. 334412)
WHEREAS, the City of Anaheim has a Long Term Disability
Insurance Policy written by Standard Insurance Company; and
WHEREAS, it is desirable to make certain amendments with
respect to coverage under said policy; and
WHEREAS, there will be no cost for this benefit modifi-
cation for the balance of this Policy year ending December 31,
1979; and
WHEREAS, an amendment to said policy has been prepared,
the terms and conditions of which are acceptable to the City
Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and condi-
tions of said Amendment are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Human Resources Director
be, and he is hereby, authorized and directed to execute said
Amendment on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the
City Council of the City of Anaheim this 17th day of July, 1979.
