1979-721WHEREAS, the City of Anaheim has heretofore entered
into an agreement with Kaiser Foundation Health Plan, Inc.; and
WHEREAS, the parties desire to amend said agreement
with respect to certain benefits and the premium structure; and
WHEREAS, the terms and conditions of said Amendment
are acceptable to the City Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said Amendment are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Human Resources
Director be, and he is hereby authorized and directed to execute
said Amendment on behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by
the City Council of the City of Anaheim this 11th day of
December, 1979.
