A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A MASTER GROUP AGREEMENT
FOR EMPLOYEE HEALTH CARE BY AND BETWEEN THE
CITY OF ANAHEIM AND GENERAL MEDICAL CENTERS
HEALTH PLAN, AND AUTHORIZING THE HUMAN
RESOURCES DIRECTOR TO EXECUTE SAID AGREEMENT.
WHEREAS, an agreement was entered into by and
between the City of Anaheim and General Medical Centers
Health Plan on April 18, 1977; and
WHEREAS, General Medical Centers Health Plan
has changed its prepayment fees to become effective
January 1, 1980; and
WHEREAS, the parties desire to enter into a new
Master Group Agreement which will reflect such rate changes
to be effective January 1, 1980; and
WHEREAS, an agreement has been prepared, the
terms and conditions of which are satisfactory to the City
of Anaheim.
NOW, THEREFORE, BE IT RESOLVED by the City Council
of the City of Anaheim that the terms and conditions of a
Master Group Agreement, by and between the City of Anaheim
and General Medical Centers Health Plan, be, and the same
are hereby, approved.
BE IT FURTHER RESOLVED that the Human Resources
Director be, and he is hereby, authorized and directed to
execute said agreement for and on behalf of the City of
Anaheim.
THE FOREGOING RESOLUTION is approved and adopted
by the City Council of the City of Anaheim this llthday of
December, 1979.
