A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A RENEWAL AGREEMENT WITH
FAMILY HEALTH PROGRAM, INC. FOR EMPLOYEE HEALTH
SERVICES AND AUTHORIZING THE HUMAN RESOURCES
DIRECTOR TO EXECUTE SAID RENEWAL AGREEMENT ON
BEHALF OF THE CITY OF ANAHEIM.
WHEREAS, the City of Anaheim and Family Health Program, Inc.
have previously entered into agreements whereby Family Health
Program, Inc. provided health services for employees of the City; and
WHEREAS, Family Health Program, Inc. has submitted a
Renewal Agreement to the City to provide for such health services
effective January 1, 1980; and
WHEREAS, the terms and conditions of the Renewal
Agreement are acceptable to the City Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and
conditions of said agreement are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Human Resources Director
be, and he is hereby, authorized to execute said Agreement on
behalf of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by
the City Council of the City of Anaheim this 11th day of December, 1979.
