RESOLUTION NO. 79R -716
A RESOLUTION OF THE CITY COUNCIL OF THE CITY
OF ANAHEIM APPROVING A RENEWAL AGREEMENT WITH
CALIFORNIA MEDICAL GROUP HEALTH PLAN, INC.
FOR EMPLOYEE DENTAL SERVICES AND AUTHORIZING
THE HUMAN RESOURCES DIRECTOR TO EXECUTE SAID
RENEWAL AGREEMENT ON BEHALF OF THE CITY OF
ANAHEIM TO BE EFFECTIVE JANUARY 1, 1980.
WHEREAS, the City of Anaheim and California Medical
Group Health Plan, Inc. have previously entered into agreements
whereby California Medical Group Health Plan, Inc. provided dental
services for employees of the City; and
WHEREAS, California Medical Group Health Plan, Inc. has
submitted a Renewal Agreement to the City to provide for such
dental services effective January 1, 1980; and
WHEREAS, the terms and conditions of the Renewal
Agreement are acceptable to the City Council of the City of Anaheim.
NOW, THEREFORE, BE IT RESOLVED that the terms and conditions
of said agreement are hereby approved and accepted.
BE IT FURTHER RESOLVED that the Human Resources Director
be, and he is hereby, authorized to execute said Agreement on behalf
of the City of Anaheim.
THE FOREGOING RESOLUTION is approved and adopted by the
City Council of the City of Anaheim this 1 day of December, 1979.
