Pursuant tothe state statutes regarding public information, Iam inquiring towhether you can provide the following
information:
1. Acopy ofany records related to uncashed /stale-dated checks showing the (i) payee orvendor names, (ii) check
issue dates, (iii) check number and (iv) dollar amounts that have aged more than sixmonths from the date ofthisletter
inanamount equal toorgreater than one thousand dollars ($1,000.00).
2. Accounting records ofproperty tax overpayments orclaimed/unredeemed tax liencertificates which have been
refundable for more than sixmonths from the date ofthis letter showing the (i) payee names (ii) check issue dates, (iii)
check numbers, (iv) dollar amounts over $1,000.00.
3. For the above two requests please include allthe necessary claim forms, affidavits orinstructions required for the
reissuance ofthe deposits/outstanding/stale dated checks orrefunds.
4. Atwhat frequency are these records updated? Monthly, quarterly, semiannually, annually orupon request? Please
confirm, ifuncashed checks are remitted tostate unclaimed property bureau. Ifso, after what aging period?
