Purpose:
This Regulation establishes procedures for reporting employee work -related serious injuries,
serious illnesses, and fatalities to Cal/OSHA.
Policy:
The City of Anaheim, in accordance with state law, will immediately notify Cal/OSHA by
phone of any work -related employee serious injury, serious illness or death. Cal/OSHA
defines "immediately" to be as soon as practically possible but not longer than eight (8) hours
after the employer knows, or with diligent inquiry would have known, of the incident.
A serious injury or illness is defined as one in which (i) requires inpatient hospitalization of an
employee (for other than observation), (ii) suffers an amputation, the loss of an eye, or (iii)
incurs a serious permanent disfigurement.
Responsibilities:
The Fire Department shall use the Anaheim Fire Department Lexipol Policy 904-
Cal/OSHA Notification of Injury Illness and Death, Cal/OSHA Notification Procedure, to
report any City of Anaheim employee work -related serious injury, illness or fatality, or
other work -related serious injury, illness or fatality in the private and public sectors to
which they respond.
City departments shall:
1. Report serious injuries, serious illnesses, and fatalities by phone to the Risk Management
Division within two (2) hours of the time the department becomes aware of an incident and
provide the information listed in the CALLING CAL/OSHA paragraph on page 2.
2. If the Anaheim Fire Department does not respond to an employee serious injury, illness
or fatality incident, and if the Department is unable to contact someone from Risk
Management within two (2) hours, the Department shall immediately notify Cal/OSHA and
provide the information listed in the CALLING CAL/OSHA paragraph on page 2.

The Risk Management Division will provide assistance and guidance to departments in their
response to a serious injury/illness or fatality.
PROCEDURES IN THE EVENT OF A SERIOUS INJURY OR ILLNESS
Departments shall institute the following initial actions:
1. See that first aid and follow-up medical care is being provided for the injured person(s).
2. Attempt to correct or isolate the hazardous condition that caused the injury/illness so that
additional people are not injured.
3. Have department management obtain written statements from witnesses (as quickly as
possible so events are clear in people's memories).
4. Contact the responding emergency agency (Police/Fire) and request a copy of their incident
report (i.e., Police: APD Incident Report; Fire: California Incident Reporting System
report).
5. Review A.R. 255, Reporting On -the -Job Accidents, and A.R. 273 Cal/OSHA Inspection
Policy.
CALLING CAL/OSHA
When phoning Cal/OSHA (24-hour reporting number: 714/558-4451) to report a serious injury,
serious illness, or death, be prepared to provide as much of the following information as
possible:
1. Time/date of accident
2. Department/address/phone number
3. Name & job title of reporting person
4. Incident site address
5. Contact person at the site
6. Name/address of injured
7. Nature of injury
8. Current location of injured
9. Names of police/fire agencies responding
10. Description of accident & whether scene has been altered
The department should notify the Department Designated Representative (reference: A.R. 273)
for Cal/OSHA Inspections as soon as possible any time there is a serious injury/illness or fatality
since Cal/OSHA may respond to such an incident.
